The Carriage House Campus is a community devoted to Senior Living, encompassing Independent and Assisted living, Short-term rehabilitation and Long-term care. The Carriage House is a 165-bed Medicare/Medicaid-certified health and rehabilitation center.

Occupational Therapy
We provide supervision or assistance with activities of daily living (ADLs) and monitoring of resident activities to help to ensure their health, safety, and well-being.
Rehabilitation
The Carriage House's Rehabilitation Program is involved in the treatment of disorders of the musculoskeletal system and it’s interaction with physical movement. 
Community
The Carriage House also provides complete skilled nursing and rehabilitation services to meet the post-hospital needs and long-term care needs of our patients and residents.

Elmer Ayres
Elmer was born on June 19, 1933 in Beaver Township in Bay City, MI to his mother, Martha Sauer. She later wed Wes Ayres, who adopted Elmer when he was 14 years old.

Pet Pass

We love visitors, including those with four legs. For pets to gain access to the facility, the Activities Department will need up-to-date shot records on file.


Getting Ready for Summer

It is shaping up to be a beautiful spring and summer is right around the corner!

To keep our residents comfortable we ask that your loved ones wardrobe is weather appropriate.  Airy tops and light jackets are great for patio time and the warmer days that are ahead of us.

Physical Therapy
Physical Therapy is involved in the treatment of disorders of the musculoskeletal system and its interaction with physical movement.

The ultimate goal of physical therapy is to restore maximal functional independence to each individual.


Speech-Language Therapy
Speech-language pathology is the study of disorders that affect a person’s speech, language, cognition, voice disorders, swallowing disorders
and the rehabilitative or corrective treatment of physical and/or cognitive deficits/disorders resulting in difficulty with communication and/or swallowing.


Pain Management
The Carriage House Health & Rehabilitation Center provides a comprehensive pain management
team approach which evaluates, treats and determines individualized interventions to provide pain relief,  manage chronic pain, improve strength and endurance to assist in returning residents to their maximum level of functioning.
